By checking the box below, I voluntarily authorize the client company named above (or the staffing organization that administers my pay) to deduct from my wages each pay period the monthly contribution for my selected plan, and to remit that amount to the Remote Universal Salary pooled reserve on my behalf. This authorization covers only my plan contribution and no other sum. It stays in effect until I revoke it in writing, change my plan, or my enrollment ends — revoking it does not refund contributions already remitted. My employer's role is limited to withholding and remitting this amount; it is not the insurer, guarantor, or administrator of the reserve.

Onboarding here does not collect a digital payroll deduction authorization. Have the member complete and sign the Enrollment Packet & Participation Agreement (Section 4) on paper, or direct them to register themselves online, which captures authorization electronically.
